Book excerpt: Based on documented studies, education provided for dialysis patients on diet and fluid restrictions improved patient outcomes. The author has intended to improve patient outcomes of dialysis patients through a nurse-lead education program. Patients are educated once a month by a Registered Dietician (RD) which is not a sufficient amount of education needed to improve patient outcomes. A nurse-lead education program will help non-compliant patients and those who need help with diet and fluid restriction, meet target goals for improved patient outcomes. According to Brogdon, (2013), a pilot study done on improving patient knowledge of phosphorus using an educational tool such as a brochure showed improved patient outcomes. Evidence-based research showing improved outcomes of ongoing education will be provided to stakeholders as well as a detailed plan of a goal-setting teaching plan to patients lead by nurses. The nurse-lead diet and fluid education program is in the planning stage. Upon dissemination and evaluation of the proposed program, end results will be evaluated with stakeholders and adjusted accordingly.
