Book excerpt: Hyperphosphatemia in the hemodialysis (HD) population is common, and independently correlated to increased morbidity and mortality. This makes it an especially salient issue as it is a modifiable variable in a population with an average yearly mortality rate of approximately 20%. The registered dietitian (RD) working in an out-patient dialysis facility has the potential to impact this important clinical outcome through health counseling and nutritional education. After considering the limitations of the population and the average caseload of a renal RD, a more innovative mode for repetitive education is needed. A pilot study was conducted with in-patient hemodialysis patients in center-city Philadelphia using a brief, pre-recorded educational piece independently developed and presented by the researcher, a renal RD, shown at chair-side during HD treatment. The researcher also designed a pre-/post-test using ten true/false questions for ease and clarity; twenty-four subjects provided usable data sets. Results were analyzed using simple statistics, and supported the researcher's hypothesis that this mode of educational intervention is possible to increase patient's knowledge related to nutrition parameters. Further exploration and development within this vein of nutrition education should be continued. Although this study had many limitations, there is potential for this type of educational intervention to be used as a repetitive educational aid in the chronic hemodialysis population to attenuate hyperphosphatemia.
